Under the National Implementation and Dissemination for Chronic Disease Prevention initiative, the Directors of Health Promotion and Education (DHPE) is providing technical assistance, training and resources to 50 selected communities  that are receiving funding for improving health, preventing chronic disease and reducing health disparities.

Overview

To support health promotion efforts at the community level, the U.S. Centers for Disease Control and Prevention’s Division of Community Health (CDC/DCH) awarded cooperative agreements to three national organizations that will provide sub-awards to chapters/affiliates. The three national organizations are the American Heart Association, National WIC Association, and the American Planning Association.Two other national organizations – DHPE and the Society for Public Health Educators (SOPHE) – received awards to provide training and technical assistance to the chapters/affiliates. The chapters/affiliates will build and strengthen their community’s ability to use population-based strategies that create healthier communities. The goals are to reduce tobacco use and exposure, improve nutrition, increase physical activity, and improve access to chronic disease prevention, risk reduction, and management opportunities.

Goals

The overall goal of the National Implementation & Dissemination for Chronic Disease Prevention Project is to implement, evaluate, and disseminate evidence- and practice-based community health activities that will build local-level capacity and implement population-based strategies to prevent and reduce chronic diseases, reduce health disparities, and develop a
stronger evidence-base for effective prevention programming. The impact of this project will be:
  • A 5% reduction in the rate of death and disability due to tobacco use
  • A 3% reduction in the prevalence of obesity and
  • A 3% reduction in the rates of death and disability due to diabetes, heart disease, and stroke.

National Organizations

Five national organizations received funding from the Centers for Disease Control and Prevention under two categories.

Category A national organizations participating in this project are American Heart Association, American Planning Association and National WIC Association. They will provide grants to 15-20 chapters/affiliates for capacity building and public health work at the community level.

Category B national organizations participating in this project are the Society for Public Health Education (SOPHE) and the Directors of Health Promotion and Education (DHPE). They will provide training, resources, and support to the chapters and affiliates of the Category A national organizations and their community coalitions; and disseminate and communicate messages about prevention, successes, and the progress of work under this national initiative.

Focus Areas

Sub-recipient chapters/affiliates will target their efforts in the following focus areas: reducing tobacco use and exposure, improve nutrition, increase physical activity, and improve access to chronic disease prevention, risk reduction, and management opportunities.

Each Category A national organization and its chapters/affiliates will select one of more of focus areas.

American Planning Association: nutrition, physical activity

National WIC Association: nutrition, access to chronic disease prevention, risk reduction, and management opportunities
